Madonna owned the 2026 MTV Video Music Awards from open to close, kicking off the ceremony with her first VMAs performance in 23 years and ending it by taking home Artist of the Year, the show's marquee honor.

The 67-year-old opened the broadcast at the Peacock Theater in Los Angeles sprawled spread-eagle atop a black grand piano for a medley of "Bring Your Love" and "Danceteria Afterhours," with Sabrina Carpenter playing the keys beneath her and Charli XCX joining the number. It was a full-circle moment for an artist who first performed at the VMAs in 1984 and hadn't returned to that stage in over two decades.

Madonna arrived at Sunday's ceremony with 13 nominations, the most of any artist, and left with two wins: Artist of the Year and Best Collaboration for "Bring Your Love" with Carpenter. She also picked up Best Dance for "Confessions II — The Film." For Artist of the Year, she beat out Ariana Grande, Bruno Mars, Morgan Wallen, Carpenter herself and Taylor Swift — who was instead honored separately with the ceremony's Artist-Director Honors.

That was Madonna's acceptance speech in full, delivered to a standing ovation inside the theater. The show, which aired simultaneously on MTV and CBS and streamed on Paramount+ from 7:30 to 9:30 p.m. ET, also handed the Video Vanguard Award to Nirvana and named LISA's "Dream feat. Kentaro Sakaguchi" Best Pop video.

Madonna's win extends a career that has now spanned five decades of VMAs relevance, dating back to the ceremony's second-ever broadcast. Sunday's performance and speech were widely read as a victory lap for an artist who has spent much of the past two years fending off questions about her health and touring future after a 2023 ICU hospitalization forced the postponement of her Celebration Tour.
